Tax Sheltered Annuity (OMNI) Deductions should be based on 10 mos - 20 pays and 12 mos - 24 pays
Click Link OMNI Forms, Blue box left-hand side scroll down to New York, and type in Poughkeepsie City School District, this will bring up a list of providers and phone numbers to choose from, once you have decided on a provider on the right-hand side under "Sign Up or Change Your Contributions" to begin the process online.
