    Welcome to Clinton Elementary School! We hope your child’s experience here is a healthy and prosperous one. We would like to take this opportunity to explain some rules of the School Health Office and ask for your help to maintain a safe environment.


    Please note that all contact information NEEDS to be updated. Along with an emergency contact in case we can NOT reach our parents. 

     

    Special Health Needs:

    Please communicate any special health needs or problems to the school nurse (ex: asthma, allergies (food, bees stings), difficulty with hearing, vision or speech, medications, accidents, stitches, contagious or serious illness, or surgery).  The need for special aid such as a wheelchair or crutches requires a medical doctor’s order.

    Medication in School:

    If your child must have medication, prescribed or OTC, while in school or at a school activity, the following must be met:

    - The school must have on file a written order from a duly licensed physician. Verbal or telephone orders are not acceptable.

    - Written permission from the parent or guardian.

    - The medication must be in a properly labeled container and brought in by the parent to be stored in the health office.

    - If the student is going to carry and self-administer the medication, the order must state that they may carry and self-administer the medication.

     

    ***Please note: A new medication order must be submitted each school year regardless of past use. Medication forms are available from the school nurse.

    Immunization:
    All students in New York State are required to be properly immunized before entering. Please see a list of required immunizations at www.health.ny.gov.
     
    School Physicals:
    New York State requires annual Physical Examination & Vision & Hearing screenings for students in Kindergarten, 1st grade, 3rd grade, 5th or for those newly enrolled in the district.
